Creating a Policy Form
In order to issue a policy through the DAIS policy system, a policy form must be added. This can be done using the following steps:
1. Click into the Product Builder, and 'Configuration'
2. Select 'Add Form'
3. Choose 'Policy' from the list of form types
4. Give the form a name in the 'Policy Name' box
5. Select 'add and map form' to load an existing PDF file and choose which question fields to map to the Policy Form.
Be sure to save your mappings, then the product configuration when you are finished making any changes.
The policy form can now be attached to the Payment Setup for the product.
